The role of the RN Team Leader is the coordination, planning, and delivery of resident care based on established interdisciplinary plans of care using the nursing process.
The RN Team Leader provides clinical leadership and direction to the CNAs on the team to assure resident care is provided in accordance with the established policies and procedures and within professional practice standards to assure quality resident care is achieved and that federal, state local, and Joint Commission regulations are met.
The RN Team Leader practices within the scope of a professional nurse clinically and also administratively as the need arises.
All assigned tasks are carried out in accordance with policy and procedures of Fellowship Manor and the Mission and Core Values of Fellowship Community.
ESSENTIAL QUALIFICATIONS / CREDENTIALS A minimum of three (3) years of experience in a long-term care or equivalent nursing leadership capacity.
Knowledge of federal, state, Joint Commission, and local skilled nursing regulations.
Current Pennsylvania licensure as a Registered Nurse.
Must be able to read, write, comprehend, and speak English.
Intermediate computer skills required Ability to organize, prioritize and provide direction in the care of residents.
Experience in nursing care of the geriatric resident and the geriatric resident with dementia.
Demonstrates effective leadership skills in delivery and coordination of resident care.
Possess the confidence and the initiative to respond to emergent situations.
Ability to prioritize, organize and address demanding needs as they arise.
Experience in performance reviews and HR laws as relates to personnel management Demonstrated effective problem-solving and clinical decision-making skills.
Certified in Basic Life Support.
Must be able to see adequately to perform visual and qualitative physical and behavioral assessments of Geriatric residents and geriatric residents with dementia.
Must be able to communicate with persons who may be visually and/or hearing impaired.

PHYSICAL REQUIREMENTS Occasional sitting Frequent bending, standing, push/pull over 50 pounds, carrying up to 25 pounds, lifting over 40 pounds, stooping, reaching, and stretching Constant walking, hearing, and seeing ESSENTIAL DUTIES AND RESPONSIBILITIESNursing / Resident Care Consult with residents, responsible party, Nurse Manager / Supervisor in planning and providing resident's care, treatment, rehabilitation, and discharge.
Notify Nurse Manager / Supervisor and responsible party of change in resident's condition.
Provide professional nursing care using nursing process.
Perform comprehensive resident assessments as required.
Assist to admit, transfer, and discharge residents as necessary and directed.
Follow established infection control and OSHA guidelines in accordance with facility's policies and procedures.
Review resident's chart for specific treatments, medication orders, diets, etc.
as necessary.
Support a restraint-free environment for residents.
Communicate with residents and family members as necessary and report significant concerns to Nurse Manager / Supervisor.
Participate and support restorative and rehabilitation program objectives Ensure that personnel providing direct care to residents are performing in accordance with facility's policies and procedures, resident's care plan, and resident's wishes.
Receive report at beginning of shift; give report at end of shift.
Inform nursing personnel of appropriate information regarding new admission and ensure that resident's room is prepared for new admission.
Participate in the orientation of new residents and family members to facility and procedures.
Oversee nursing care to ensure that all residents are treated fairly and with kindness, dignity, and respect.
Ensure that all nursing care is provided in privacy and that nursing service personnel knock before entering resident's room.
Ensure that all nursing personnel are knowledgeable of the resident's rights and responsibilities including the right to refuse treatment.

The check will bounce and the victim is left responsible.
